PAF Co-Pay Relief (CPR)

Description:

Program provides direct co-payment assistance for pharmaceutical products to insured Americans who financially and medically qualify. You must be diagnosed with and taking medication(s) for one of the following conditions: breast cancer, lung cancer, lymphoma and cutaneous t-cell lymphoma, prostate, kidney, colon, pancreatic, head/neck cancers, malignant brain tumor, sarcoma, diabetes, multiple myeloma, myelodysplastic syndrome (and other pre-leukemia diseases), osteoporosis, pain, hepatitis C, rheumatoid arthritis, selected autoimmune disorders.
